New Patient Registration
The practice is currently welcoming new patients who wish to register with practice permanently. The practice will accept patients from Burntwood, Heath Hayes and Norton Canes area.
If you would like to register with our practice, please bring your medical card to the surgery if you have one available.
You will need to complete our permanent patient registration form and a health information form which are both available from reception or can be downloaded from the above "Registering Online" tab.
In order to complete the registration process you will be asked to make an appointment with our healthcare assistant who will complete a new patient health check, this will include a urine and blood pressure check. If you are taking any regular medication it would be helpful if you brought along your repeat medication slip to your appointment.
If you have any long term medical conditions you may need to see another clinician for a full assessment to be undertaken to ensure your condition is being monitored and that your are receiving the help and support need for managing your condition.
